Exposure Hierarchy

This printable Exposure Hierarchy Worksheet supports gradual exposure work for anxiety, phobias, and other avoidance-based patterns. It guides you through identifying feared situations, rating distress levels, and creating a step-by-step plan that feels manageable and safe.

You are encouraged to move at your own pace and only complete the sections that feel manageable. If anything becomes too intense, you can pause at any time. We can explore them safely and collaboratively in session at a pace that feels right for you.
